Filing Social Security Disability Forms
Here are the forms used to apply for disability benefits through the Social Security Administration.
SSA-16-BK, Application for Disability Insurance Benefits Application for Disability Insurance Benefits (SSDI only)
SSA-8000-BK, Application for Supplemental Security Income (SSI) Application for Supplemental Security Income (SSI only)
SSA-3368-BKAdult Disability Report
SSA-3369-BK Work History Report
SSA-827, Authorization to Disclose Information to the Social Security Administration
SSA-3441 Disability Report Appeal,
Form SSA-561, Request for Reconsideration, if you are requesting a reconsideration of your claim;
Form HA-501, Request for a hearing Administrative Law Judge, if you are requesting a hearing.
If you are filling out the form yourself, make sure you are using a current version of the forms which all can be found on SSA.gov. Each form should have a revision date in the bottom left-hand corner. If the revision date is not within the last year or two, or if you are unsure that you have the correct form, contact your local Social Security office. Forms should be completed using either black or blue ink, and if you are printing the information on the forms by hand, take care to make sure your form is neat and legible. If you file online, you don’t have to worry about this (online filing is available only for Social Security Disability Insurance benefits, not SSI benefits).
You can have a friend or family member assist you in completing the forms, but make sure that all of the questions are answered as if you (the person applying for the disability benefits) were answering the questions in other words using first person or I.
Note that the Adult Disability Report form should be completed by you, not your doctor. Doctors will fill out medical source statements or MSS.
